The inquiry form
When you click Send, your name, email address, optional organization and phone number, message, and related technical request information are sent to FormSubmit, our form delivery provider, and forwarded to claudiushealth@gmail.com. We use the information to respond to your inquiry. FormSubmit states that it retains submissions for 30 days. Our email provider also processes and stores the resulting email.
